Yes. You can change Support at Home providers if another organisation is a better fit for your needs, preferences or budget.

Before changing, check your current service agreement for:

  • any required notice period

  • cancellation arrangements

  • outstanding fees or final charges

  • how your records and care information will be transferred

  • whether any equipment, subscriptions or subcontracted services need to be dealt with separately.

It is usually best to choose and confirm your new provider before ending services with your existing provider. This reduces the risk of essential support being interrupted.

Ask the new provider:

  • when services can start

  • whether suitable workers are available

  • whether it can provide the same services and schedule

  • what its prices and minimum visit times are

  • who will coordinate the transfer

  • whether it needs updated assessments, care plans or health information.

Agree on a clear final date with your current provider and a clear commencement date with the new provider.

The outgoing provider should stop claiming from your Support at Home budget from the agreed end date, provide a final statement and transfer relevant care information with your consent.

The incoming provider must connect you to the correct service branch and complete its onboarding process before it can begin claiming for services.

You may need to help coordinate the transfer of:

  • your care plan

  • service schedule

  • medication or clinical information

  • risk assessments

  • emergency contacts

  • worker preferences

  • equipment or assistive technology arrangements.

Do not cancel essential services until the new provider has confirmed that it can begin. This is particularly important for personal care, nursing, meal support and other services that cannot safely be interrupted.

Changing providers should not cause you to lose your Support at Home funding, but poor coordination can create service gaps, duplicate bookings or incorrect claims. Ask both providers to confirm the changeover dates in writing.
